This issue happens when the iPad thinks that it should open the Lexia webpage versus actually going to the app. In iOS 13, there is a setting under Safari that says "Pull from app" or "Don't pull from web" or something like that. (I don't have that version so I couldn't see it. In iOS 12, go to Safari -> Advanced -> Website Data -> Edit -> Delete lexiacore5.com -> Done.
